OBTAIN A REPEAT PRESCRIPTION?

We operate a computerised repeat prescription system for patients taking regular medication.
A repeat prescription may be ordered by post enclosing an SAE, by posting your repeat prescription slip in the box provided at reception or, if you prefer, ordering it on-line. (The
receptionist will need to register you to obtain an access ID.)
Some of the local pharmacies run their own schemes in which they will manage your repeat prescriptions for you and provide collection and delivery free of charge.
Every few months your doctor will need to see you, to see if any changes need to be made to your medication.

HOW DO I...
OBTAIN TELEPHONE ADVICE?
The doctors can often give advice on the telephone. They now have allocated daily time slots for telephone consultations. The receptionists will advise the most suitable time for you to speak to the doctor. This is a
daily service. We also operate a morning on-call service where you can speak to a GP who can deal with your medical needs over the phone or when appropriate book you an appointment to attend the surgery.

MAKE AN APPOINTMENT?
Please note that we do not offer a 'walk-in' service - appointments must be booked. For urgent matters that you need to be dealt with that day, please phone and ask to speak to the on-call doctor who will decide accordingly.

- Select Option 3 on our phone system to book an appointment with the doctor or nurse.
- Alternatively, you can book and cancel appointments on-line - ask at reception for your password and log-in details.
- Between 8.00 - 10.00am Monday to Friday you can queue for a telephone consultation (see above) with the on-call doctor, by selecting Option 4 on our phone system.
- Throughout the week each doctor offers telephone talk times when you can ring and queue for a telephone consultation - ask reception for the schedule.
- When you attend your appointment you can book yourself in by using our touch screen in reception.

Our Promise To You
- We will routinely offer you an appointment within 48 hours of your request. We cannot guarantee this will always be at a convenient time for you or be with your preferred choice of doctor or nurse.
- We will deal with your appointment request in one telephone call; you will not be asked to ring back another day in order to book that day's appointments.
- You can book appointments with a doctor or nurse up to six weeks in advance
- Repeat prescriptions will be ready for collection 24 hours after the request is placed.
- All urgent cases will be dealt with on the same day by our on-call doctor. This will usually involve a telephone consultation in the first instance.
Your Promise To Us
- Please cancel appointments if you no longer need them so we can offer them to patients who do. Should you repeatedly fail to attend or cancel appointments, we will have no alternative but to remove you from our
patient register.
- Please arrive on time for your appointment.
- Please book double appointments if you want two family members to be seen at the same time, and more for family members.

OBTAIN A HOME VISIT?

These are for patients who are housebound or whose condition would deteriorate by travelling. We DO NOT offer this service for childcare problems, transport problems or financial difficulties. We expect children to be
brought to the surgery. They can be seen quickly and in the most appropriate surroundings, where action can be taken immediately if needed. We have appointments available every day and we will see your child at short
notice. Advanced Nurse Practitioner Sister Lynn Kitson deals with the majority of the home visits as she specialises in long-term conditions, palliative care and care of the elderly.

OBTAIN A SICK CERTIFICATE?
For your first week of sickness you do not need to see a doctor for a sick note. This can be obtained from your employer and is called an SC2. If your employer insists on a doctor's sick note for the first seven days there will be a charge.
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work' (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, eg a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - Fit Note
The 'fit note' was introduced on 6th April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury. For more information go to www.direct.gov.uk

REGISTER WITH THE PRACTICE?
New patients are asked to complete a lifestyle questionnaire when applying to join our list. We also like all new patients to have a
health check with the nurse. This helps to ensure that we understand your health needs so we can provide the right care for you. You can, of course, see the doctor if you have an urgent problem.
The practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

CHANGE MY DETAILS?
Please notify us of any change of your personal details so that we can amend our records; it is essential that we have your telephone number as we may need to contact you. (Remeber you can do this online if you wish.)In order to provide the level of service we
want, and to stop our list from increasing too much, we have had to draw up a practice boundary which will limit the distance that we can travel for home visits. In some cases when you move house this will take you outside
our practice area. In these circumstances we will unfortunately have to ask you to re-register with another practice nearer to your new home.
